Working within a multi-disciplinary team, provides comprehensive case management services to improve mental health, and in conjunction with the primary care provider, improve medical health to adults with serious and persistent mental illness and children with serious emotional disturbance.

Qualifications:

  • Bachelors Degree in Human Service or related field required
  • Certified as a Mental Health Rehabilitation Technician (MHRT) or eligible to obtain within a reasonable amount of time as discussed with supervisor.
  • Basic Life Support (BLS) certification is to obtained prior to independently caring for patients and is to be kept current.
  • Valid vehicle operator’s license in good standing. With access to a vehicle insured with at least the minimum insurance coverage required by state law.
