This is a unique opportunity to join a growing team of dedicated behavioral health professionals! As a Care Coordinator, the employee will provide person-centered and recovery oriented care management and coordination to eligible MassHealth consumers served in the community. The Care Coordinator will be responsible for proving outreach to and engaging eligible consumers in services, by assisting consumers to identify their individualized care goals, identify appropriate community based resources, communicate with the multi-disciplinary team, and completing ongoing need assessments, among other duties as identified. This is a great opportunity to gain experience with community outreach, and providing coordination for consumers with medical and behavioral health needs . The Care Coordinator will also help people served to utilize available community based resources and maintain a healthy quality of life. Excellent part time schedule available! This position requires travel between and within the Blackstone Valley, North Shore and Norwood area.
