Position Summary: The Nurse is responsible to obtain the health history and provide nursing care to meet the needs of an assigned group of residents/patients within the framework of the total plan of care.

Must have a current valid New Jersey Nursing License
Responsibilities / Accountabilities

2. Assesses, plans, implements and evaluates a resident/patient plan of care, assigning nursing diagnosis and documenting nursing orders to direct the care.
3. Performs direct services for resident/patient such as administrating medications and providing treatments, providing personal care to facilitate continual assessment and evaluation, and assisting with diagnostic and therapeutic procedures.
4. Coordinates the communication of resident/patient care needs with members of the other disciplines utilizing an in-depth knowledge base to ensure comprehensive follow-through on issues.
5. Compares and interprets previous and present data and information; reports and records pertinent data; recognizes, corrects, and reports safety concerns.
6. Identifies and delegates suitable components of care to LPN and CNA; supervises, educates and provides guidance as necessary.
7. Plans and initiates resident/patient and family health education.
8. Utilizes knowledge base of group dynamics when working with residents/patients and participating in staff and interdisciplinary meetings.
9. Functions independently with minimal direction from the unit manager; assumes charge and staff nurse responsibilities as assigned.
10. Performs tasks that require the expertise of a registered nurse.
11. Assumes responsibility for own professional growth and development; plans and/or participates in conferences and classes to improve knowledge and practice in resident/patient care; attends in-services programs that are provided to facilitate compliance with regulations; completes annual training as assigned in a timely manner.
12. Participates in the nursing quality assurance programs and utilization meetings .
13. Maintains and promotes cohesive public and professional relationships, including community, resident, patient, family and physician services.
14. Concerns his/herself with the safety of all Center residents/patients in order to minimize the potential for fire and accidents. Also, ensures that the Center adheres to the legal, safety, health, fire and sanitation codes by being familiar with his/her role in carrying out the Center’s fire, safety and disaster plans.
15. Acts as a liaison for residents, families, visitors and staff to resolve concerns an issues, always with the goal of customer satisfaction.
16. Participates in resident/patient care conferences as they relate to resident/patients on the nursing unit.
17. Ensures that resident/patient rights are protected.
18. Puts Customer Service First: Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights.
19. Performs other duties as requested.
